ABSTRACT

Objective To understand the cognition and influencing factors of hand hygiene (HH) among healthcare-associated infection (HAI) management staff in Gansu Province, and provide reference for enhancing the special competency of HAI management staff.Methods According to geographical area, natural and human characteristics, HAI management staff in 69 hospitals in different areas of Gansu Province were selected to conduct a questionnaire and field survey on their basic condition and knowledge of HH, current situation, and influencing factors for HH knowledge were analyzed.Results Among 69 surveyed hospitals, 82.61% were secondary hospitals, 17.39% were tertiary hospitals;among 125 surveyed HAI management staff, 82.40% were full-time staff and 17.60% were parttime staff.33.60% and 66.40% of HAI management staff had complete and incomplete knowledge of HH respectively;the knowledge about importance of HH, use of hand disinfectant, installation of HH facilities, hand washing methods, hand drying methods, HH indications, and standard for judging qualified HH in tertiary hospitals were all higher than those in secondary hospitals (all P<0.05).Multivariate logistic regression analysis showed that independent protective factors for HH cognition were working for≥5 years and<10 years in HH management (OR=3.067), bachelor's degree (OR=3.331), and full-time job (OR=10.309).Conclusion HAI management staff's cognition on HH among is low, especially those who have been engaged in HH management for less than 5 years, whose educational background is secondary school or lower, and part-time personnel.It is necessary to combine with actual condition to strengthen professional competence training for HH management staff.

ABSTRACT

Objective To observe the application of enhanced recovery after surgery (ERAS) in complicated hepatic alveolar echinocoecosis surgery. Me thods We selected 117 patients from 2013 to 2016 and divided them into 3 groups in chronological order: Group A (n =35), the traditional management group. All patients underwent perioperative management according to previous procedures. Group B (n=44), the transitional management group. The management in the transition period was improved as far as possible in accordance with the ERAS programme. Group C (n=38), the standard process group. All patients were treated in full accordance with standard process. The hemodynamics data, fluid volume and the perioperative complications were recorded in all groups. Re s ults Compared with group C, there was no significant difference in the general condition and the incidence of complications before operation, the anesthesia time, the operation time, blood loss, blood transfusion and the heart rate in group A and B. The incidence of hunger and thirst before operation, infusion volume, urine output, the incidence of postoperative complications and the average length of hospitalization were all higher than those in group C. The differences were statistically significant. Conclus ion The clinical path optimization of ERAS in liver hydatid surgery can significantly reduce the perioperative complications and reduce the average length of hospitalization.

ABSTRACT

Objective To verify the indicator reference value ranges of blood cells analytic report by quoting the WS/T 405-2012 industry standard in Changsha area and to establish the reference value ranges of research parameters suitable for local area.Methods The blood cells analytic results in 3138 cases of reference group were analyzed according to the program of the Formulation of Reference Intervals in Clinical Laboratory Detection Items and the results were compared with the WS/T 405-2012 industry standard.Results WBC,Neu#,Mon%,MCHC,PLT,MPV,PCT,P-LCC,P-LCR and InR‰ in the report parameters and WBC-D,PDW-SD in the research parameters had no statistical differences between males and females(P>0.050);WBC,Neu#,Mon%,MCHC and PLT had no difference between genders,the indicators were narrowed compared with the reference value ranges in standard;the reference value ranges of Neu%,RBC,HGB,MCV and MCH in the male blood cells analytic indicators were narrowed,the lower limits of Bas%,Lym#,Mon# and HCT reference value ranges were elevated,the upper limits of Bas%,Lym#,Mon# and HCT reference value ranges were elevated,while the upper limits of Eos# and Eos% reference value ranges were decreased,the Lym% lower limit was decreased;Neu%,Lym#,Mon# and RBC in the female blood cells analytic indicators were narrowed,the lower limits of Bas# and Bas% were increased,the lower limits of Eos%,Lym%,HGB,MCV and MCH were decreased,the upper limits of Eos#,Eos%,Lym%,HGB,MCV and MCH were decreased and the HCT upper limit was elevated.Conclusion The Changsha area should have the reference value ranges of selective quoted standards and establishes the reference value ranges suitable for local area or laboratory.

ABSTRACT

Objective To evaluate the performance of parallel test in detecting malaria infection for returned person from malaria endemic area.Methods The blood samples of 484 returnees from malaria endemic area were analyzed and detected by thick blood smear,rapid diagnostic test (RDT) and nest PCR in four companies involving the African labor dispatching.Results The sensitivi ty of thick blood smear and RDT was 0.628 and 0.744 respectively,which of the parallel test was 0.930.On the other hand,the area under the curve (AUC) of parallel test was 0.930 (95%CI:0.895-0.986),which was higher than thick blood smear[0.814 (95%CI:0.724-0.904)]and RDT[0.847 (95%CI:0.769-0.926)].Conclusion Thick blood smear and RDT,which consist of parallel test,could improve the detection sensitivity and accuracy for returnees from malaria epidemical area effectively.This approach is worthy of popularization and application.

ABSTRACT

Objective To understand the situation of eosinophils absolute value and percentage in the population of occupational health examination in Changsha City ,and to analyze the distribution situation of physical examination abnormal results of eosino‐phils absolute value(EOS) and percentage(EOS% ) .Methods The blood cell analysis results and physical examination abnormal manifestations in 22 135 cases of occupational health examination were statistically analyzed .Results Among cases of EOS absolute value and EOS% increase ,the majority had the changes of EKG abnormality ,fatty liver ,etc .the minority were reactive and clonal increase ,meanwhile the cases of unknown cause eosinophils increase also accounted for a large proportion .Conclusion It is sugges‐ted that a large number of experiments and clinical studies should be carried out for further investigate the significance of eosino‐phils increase in the occupational health examination .

ABSTRACT

Objective To investigate the infection and distribution situations of diarrheogenic Escherichia coli among food practi‐tioners in Changsha City and to understand the carrying status of its virulence genes .Methods Totally 258 food practitioners from 9 units of Changsha City were extracted as the research subjects .Diarrheogenic Escherichia coli was detected by adopting the real time fluorescence multiple PCR .Results The infection rate of diarrheogenic Escherichia coli was 15 .5% (40/258) ,in which EAEC accounted for 10 .9% (28/258) ,EPEC for 3 .1% (8/258) and EHEC for 1 .6% (4/258) respectively ,EIEC and ETEC were not found .Among 28 strains of EAEC ,26 strains carried uidA gene ,13 strains carried aggR+pic gene and 27 strains carried astA gene , the carrying rates were 92 .9% ,46 .4% and 96 .4% respectively ;eight strains of EPEC all carried eae and uidA genes ;among 4 strains of EHEC ,3 strains carried eae gene ,moreover all 4 strains carried stx1+ stx2 gene .Conclusion Establishing a mechanism for continuously monitoring diarrheogenic Escherichia coli and deeply studying its molecular epidemiologic theory not only provide the essential data for the supervisory departments formulating and evaluating the public health measures ,but also has an important significance for evaluating the food safety status ,which can effectively control infectious source and protect the people′s health .

ABSTRACT

Objective To investigate the relationship between geranylgeranyl pyrophosphate synthase (GGPPS) gene polymorphisms and bone response to alendronate in Chinese osteoporotic women.Methods A total of 639 postmenopausal women with osteoporosis or osteopenia were included and randomly received treatment of low dose (70 mg per two weeks) or standard dose (70 mg weekly) of alendronate for one year. The six tag single nucleotide polymorphisms of GGPPS gene were identified. Bone mineral density (BMD), serum cross-linked C-telopeptide of type I collagen (β-CTX), and total alkaline phosphatase (ALP) were measured before and after treatment. GGPPS gene polymorphisms and the changes of BMD and bone turnover markers after treatment were analyzed.Results rs10925503 polymorphism of GGPPS gene was correlated to serum β-CTX levels at baseline, and patients with TT genotype had significantly higher serum β-CTX level than those with TC or CC genotype (all P<0.05). No correlation was found between polymorphisms of GGPPS gene and serum total ALP levels, as well as BMD at baseline. After 12 months of treatment, lumbar spine and hip BMD increased and serum bone turnover markers decreased significantly (P<0.01), and without obvious differences between the low dose and standard dose groups (all P>0.05). However, GGPPS gene polymorphisms were uncorrelated to percentage changes of BMD, serum total ALP, and β-CTX levels (all P>0.05).Conclusion GGPPS gene polymorphisms are correlated to osteoclasts activity, but all tag single nucleotide polymorphisms of GGPPS gene have no influence on the skeletal response to alendronate treatment.

ABSTRACT

Objective To understand the relation between smoking and serum uric acid level and to investigate whether the ser-um uric acid has the correlation with the gender,age and smoking history.Methods The data of the gender,age,blood uric acid in1 847 individuals aged 20-80 years with the healthy physical examination and without underlying diseases were performed the statisti-cal analysis.Results With male and female as the research objects,the serum uric acid level of smokers were higher than that of non-smokers and occasional smokers,the difference was statistically significant;the serum uric acid level had no statistically signifi-cant difference between smokers and occasional smokers;the serum uric acid level had no statistically significant difference among non-smoking,occasional smoking and smoking groups for males as the research objects alone;to divide the male subjects into groups according to age,the serum uric acid level of non-smokers,occasional smokers and smokers were not statistically significant among all age groups;serum uric acid level showed the increasing trend with the increase of smoking history,but there was no statistically significant difference.Conclusion The serum uric acid level of smokers is significantly higher than that of non-smokers and occa-sional smokers with male and female as the research objects;the difference in serum uric acid level between smokers and occasional smokers has no statistical significance;excluding the gender factor interference,the serum uric acid level of males is not affected by smoking or age;serum uric acid mean value demonstrates the increasing trend with the increase of smoking history.

ABSTRACT

<p><b>BACKGROUND</b>Osteogenesis imperfecta (OI) is a rare bone disease and its effective treatment is relatively deficient. We tried to assess the effects of new bisphosphonate, ibandronate on children with OI.</p><p><b>METHODS</b>In this open-label, prospective, controlled study, 30 children with OI were enrolled. They received either infusions of ibandronate (2 mg) in every three months or oral calcitriol 0.25 µg daily for 24 months. All patients took 500 mg calcium plus 200 U vitamin D daily together. The endpoints were the change of annual new fracture rate (observed by case history and X ray films of spine), bone mineral density (BMD, measured by dual energy X-ray absortiometry), serum concentration of carboxy-telopeptide cross-links of type I collagen (CTX, bone resorption marker) and alkaline phosphatase (ALP, bone formation marker) during the follow-up.</p><p><b>RESULTS</b>After the cyclic infusions of ibandronate, the annual new fracture rate was significantly decreased from 1.9 to 0.13 time, obviously lower than that of calcitriol group, which decreased from 1.8 to 1.0 time after the treatment (P < 0.001). The significant increase of BMD at the lumbar spine, femoral neck, trochanter, total hip was found in the group of ibandronate by 59.0%, 42.0%, 47.5% and 36.6% in time dependent manner (compared with the baseline, P < 0.001). The increase of BMD in ibandronate group was greater than that of calcitriol group (P < 0.001). The concentrations of ALP and CTX were obviously decreased in ibandronate group, and the reduction of CTX was more significant than that of ALP (P < 0.001). The tolerance of the children to ibandronate was quite well. Mild fever and muscle pain were found in 9 cases within 1 - 3 days after the first infusion of ibandronate, which could relieve after 1 - 2 days without special management.</p><p><b>CONCLUSIONS</b>The benefits of cyclic infusions of ibandronate to children with OI are significant because ibandronate could significantly reduce annual bone fracture rate, increase lumbar and hip BMD, preserve vertebral morphometry of patients through inhibition of bone resorption.</p>

ABSTRACT

In order to explore the effect and underlying mechanism of hypoxia on body weight, the effect of intermittent moderate hypoxia on high-fat diet-induced obesity was observed in mice, and the role of leptin in hypoxic effect was identified. Healthy Kunming mice were divided randomly into 4 groups (n=20 in each group). The control group: the mice were fed normally under the normal oxygen pressure. Hypoxia group: the mice were fed normally, and given intermittent moderate hypoxia training. Obesity group: the mice were fed diet rich in fat and sugar under the normal oxygen pressure. Hypoxia + obesity group: the mice were fed diet rich in fat and sugar, and given intermittent moderate hypoxia training. After 40 d of feeding and training, the body weight of mice was determined, and the average increasing rate of body weight in each group was calculated and normalized with food intake. Meanwhile, plasma leptin level was measured with ELISA method, and fatty degeneration and leptin receptor expression in liver were observed by Sudan III staining and immunohistochemistry, respectively. The obesity mouse model was successfully established with increases in body weight, plasma leptin level and distribution of adipocytes in the liver. The average body weight and density of adipocytes in the liver in hypoxia and hypoxia + obesity groups decreased obviously, while plasma leptin level and leptin receptor expression in the liver were increased. It is suggested that intermittent moderate hypoxia reduces body weight through elevating plasma leptin level and/or enhancing leptin receptor expression in the liver.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the changes in the serum content of immunoreactive calcitonin (iCT) after burns or inhalation injury, and to explore its diagnostic significance.</p><p><b>METHODS</b>Twenty-four dogs were randomized into 4 groups, i. e. A (n = 6, with moderate degree inhalation injury) , B ( n = 6, with severe inhalation injury), C (n = 6, with most severe inhalation injury) and D (n = 6, with severe burns) groups. The serum content of iCT and blood gas analysis before and after injury were determined at different time points. The degree of inhalation injury was determined with fibrobronchoscopic examination at 6 post-inhalation injury hour (PIH).</p><p><b>RESULTS</b>(1) Fiber bronchoscopic examination showed that the degree of inhalation injury in each group was coincident with the anticipation. (2) The serum content of iCT in each group at 1 PIH was obviously higher than that before injury, and it was evidently higher in A, B and C groups than that in D group at 4 PIH. The peak value of iCT in group A at 24 PIH was (453+/-224) ng/L, and it increased gradually in B and C groups at 48 PIH. The serum content of iCT increased continually from 2 PIH on, and it reached (125+/-41) ng/L at 48 PIH. (3) Compared with PaO2 value before injury (109+/-8) mmHg, there was no obvious difference of the PaO, in A and D groups. PaO2 value in B and C group began to descend continually at 8 PIH (65+/-6) mmHg, and that in C group began to descend at 4 PIH (71+/-9) mmHg. PaCO2 value in C group began to increase at 24 PIH(52+/-11) mmHg when compared with that before injury(38+/-5 ) mmHg.</p><p><b>CONCLUSION</b>The changes in the serum level of iCT within 8 PIH occurred much earlier than PaO2 and PaCO2, thus it has the same diagnostic significance as fibers bronchoscopic examination.</p>

ABSTRACT

Objective To investigate the effect of rosiglitazone (ROS),a peroxisome poliferator- activated receptor (PPAR)?ligand,combined with all-trans retinoic acid (ATRA) on anti-angiogenesis of transplanted gastric cancer in nude mice,and to explore the mechanism of anti-angiogenesis prelimina- rily.Methods The model of xenograft tumor in nude mice were established by inoculating human gastric cancer cells line MGC803 (lower differentiated) into the back of nude mice subcutaneously.The cancer- bearing nude mice were divided randomly into 5 groups:group 1 (n=6) with no treatment;ROS treat- ment (group 2,n=6),ROS combined ATRA treatment including:low dose treatment (group 3,n= 6),moderate dose treatment (group 4,n=6) and high dose treatment (group 5,n=6).After treated for forty days,the volume change of tumor and tumor inhibition rates were observed.The expression of CD34,vascular endothelial growth factor (VEGF) in grafts were detected by immunohistochemical and calculated the difference of MVD.The mRNA expression levels of VEGF,HIF-l?were detected by RT- PCR assay accordingly.Results①The volume of tumor was significantly decreased in ROS treatment group compared with group 1 (P<0.01).The tumor inhibition rates of group 2 were similar to group 3 (P>0.05).With the increasing of the dose of ROS the tumor inhibition rates were increased.They were dose-dependent in specified dose-range.②ROS could inhibit angiogenesis of xenograft tumor and depress expression of mRNA of VEGF and HIF-l?.When ROS combined with ATRA,the increasing of dose of ROS,inhibiting angiogenesis of tumor and depressing expression of mRNA of VEGF and HIF-l?were found (P<0.05).Conclusion ROS (25 mg?kg?2 d~(-1)) can inhibit the growth of tumor,and ROS combined with ATRA can further inhibit the growth of tumor,which may be through the path of PTEN by inhibiting the angiogenesis of tumor

ABSTRACT

Objective To clone and express HCMV UL123 gene exon 2,3(ie1-exon2,3) in bacterial two-hybrid bait plasmid,and identify its self-activation property.Methods HCMV ie1-exon2,3 carried on pTWIN1/ie1 recombinant was amplified by PCR and cloned into the pBT plasmid,which was transformed into E.coli XL1-Blue MRF' Kan host strain.The positive recombinant was identified by PCR,restriction enzyme digestions and sequencing analysis.The verified plasmid was transformed into bacterial two-hybrid system reporter strain.The soluble fusion protein was analyzed by SDS-PAGE and Western blot.The self activation effect of the recombinant was then tested.Results Bacterial two-hybrid pBT/ie1-exon2,3 bait plasmid was successfully constructed.The corresponding soluble fusion protein rIE1-N_(85)/?C1 was expressed in bacterial two-hybrid system reporter strain,and didn't show self-activation property.Conclusion Bacterial two-hybrid pBT/ie1-exon2,3 bait plasmid without self-activation property was successfully constructed,and it can be used to screen the library of fetal brains.

ABSTRACT

Objective To investigate the antiproliferation and induction differentiation of human gas- tric carcinoma which human gastric lower-differentiation mucinous carcinoma MGC-803 cells transplanted in- to nude mice by using rosiglitazone(ROS),and to preliminarily explore the mechanism of differentiation. Methods The mice were randomly divided into five groups:model,ATRA,ROS 25 mg?kg~(-1),ROS 50 mg/kg, ROS 100 mg/kg.After that the volumes were measured and inhibition rates were calculated.The cell cycle was detected by FCM.The protein expression level of Mucin SAC was detected by immunohistochemistry. Results The volume of tumor decreased significantly in ROS treatment groups,the differences had statistical significance compared with model group(P0.05).The xenograft tumors of ROS groups demonstrated the characteristics of differentiation.Xenograft tumor cells were arrested in G_0/G_1 phase,and the cells in S phase decreased significantly,and up-regulated Mucin SAC gene expression.Conclusion ROS could inhibit the growth of tumor,and the effect were dose-dependent with ROS.ROS could induce the differentiation of Xenograft tumor cells of gastric cancer.Its mechanism might be related to the inhibit of transition from G_1 to S phase,degrade the activity of proliferation,regulate the expres- sion of Mucin 5AC.
